Not only that, but, scientific evidence proves patients heal more quickly at home. Patients have reduced mortality and morbidity rates at home. Nine out of ten seniors prefer home care to institutional care. Home health care keeps families together, while encouraging patients’ independence.
Along with nursing care, transportation, nutrition and socialization are senior care concerns. Arise staff will be escorts for appointments, provide homemaker services for nutritious meals and be comforting companions for visits and overnight stays for security.
Our registered nurse creates care plans and supervises all Arise care giving staff. Our nurse has a background in public health and gerontology and intrinsically understands the needs of seniors and the concerns of their adult children.
Private Eldercare Planning, Management and Coordination
Involves advising and assisting families in determining needs of the aging adult. Caregivers navigate most of the various long-term care resources available in the community. The idea is to educate, inform and present options to the family and then assist them in implementing and monitoring those options.
An RN specially trained and certified in Geriatrics, depending on the needs of the client and family, will provide some or all of the following:
- Individualized care planning
- Screen, arrange and monitor in-home help or other services as well as residential/institutional care options
- Assistance with some forms and paperwork related to Medicare, medical assistance, insurance, etc.
- Client education and advocacy
- Family liaison when family members live at a distance from the person needing care
